2012 SPO Elimination Championships
Only online entries will be accepted.
Super Series teams register through Super Series site.

Only fully registered teams may compete in the Eliminations.
New and affiliate teams will be able to become members when they enter.

Registered teams must use their official roster.

Entry fee - $350 + HST

Please indicate your2012 status.

Registered team (Member) - paid full registration fee for 2012.

Affiliate - you have entered a 2012 SPO tournament by paying the $50 affiliation fee. ($$60.75 + 13% HST = $68.65) will be added to Elimination entry fee to upgrade to full membership)

Neither - new team ($105 + HST registration fee will be added to the entry fee.)

New SS team (Combined B & C Super Series team)($35 registration fee will be added to the entry fee.)

Do not proceed until you have your complete roster information (player's name, city and date of birth). Applications without the roster will not be accepted.
